*1. Photocopy of the Foreigner Physical Examination Form (This form is uniformly printed by the Health and Quarantine Bureau of China)

Color scan copy of the Foreigner Physical Examination Form.It must be filled in English,and the medical examinations must cover all the items listed in the Foreigner Physical Examination Form. Incomplete forms or forms without the signature of the attending physician,the official stamp of the hospital,or a sealed photograph of the applicant are considered invalid. Please determine the appropriate time to have medical examination as the medical results is valid for 6 months.

*2. Non-criminal Record Report

Non-criminal Record Report must be issued by the applicant's local public security authority,usually within 6 months before the submission date of application.

*3. Certificate of HSK

*4. Two Recommendation Letters(written in Chinese or English)

5. Other attachments

Such as award certificate,pre-acceptance notice from scholarshipin stitution,a CD of personal musical works(music program applicants),a CD of personal works(fine arts program applicants)including two sketches,two color paintings,and two other works,etc)

*6. The highest diploma must be notarized

Relevant documents in languages other than Chinese or English must be attached with notarized Chinese or English translations. Prospective diploma recipients must submit official document issued by your current school to prove your current student status or expected graduation date.

*7. Academic transcripts of high school

Documents in languages other than Chinese or English must be attached with notarized Chinese or English translations.

*8. 2-inch electronic photo

*10. Study Plan or Research Proposal

Study Plan or Research Proposal written in Chinese or English with a minimum of 500words.

*11. CSCA Test Transcript(s)

-The test covers Chinese (Humanities and STEM Chinese), Mathematics, Physics, and Chemistry. Mathematics is required for all programs, while other subjects depend on the university’s specific requirements.

Tips:
For applicants who are in China: If you are an employee, you still need to provide employment separation certificate and your current residence permit. If you are a student, you need to provide transfer certificate, leaving certificate, certificate of attendance rate issued by previous school and your current residence permit.
